TTF Course Submission Form

The following instructions are for submitting courses for the IATSE Training Trust Fund:

Fill out the form for each course you’re sending in.

Get copies of daily sign-in sheets. Your sign-in sheet must have the following information: course name, course date(s), local union, instructor name, and participants’ first and last name and date of birth.

For lift, CPR/AED, and OSHA courses, you will also need completion cards.

Email the Submission Form and sign-in sheets to tracking@iatsetrainingtrust.org.
